M\u00e5nga av studierna omfattar ett f\u00e5tal personer, och f\u00e5 \u00e4r populationsbaserade. I vissa studier inkluderas \u00e4ven personer med andra typer av spinal dysrafism \u00e4n myelomeningocele, till exempel spina bifida occulta, meningocele, lipomyelomeningocele.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>H\u00e4lsorelaterad livskvalitet*<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"font-size:18px\">Evidens g\u00e4llande h\u00e4lsorelaterad livskvalitet i denna population \u00e4r inte enhetlig<sup>1<\/sup>. Man antog tidigare att personer med ryggm\u00e4rgsbr\u00e5ck hade s\u00e4mre h\u00e4lsorelaterad livskvalitet \u00e4n personer utan ryggm\u00e4rgsbr\u00e5ck. Forskning har visat att s\u00e5 inte \u00e4r fallet<sup>2,3<\/sup>, \u00e4ven om det finns undantag<sup>4,5<\/sup>. Vissa faktorer \u00e4r associerade med reducerad h\u00e4lsorelaterad livskvalitet, s\u00e5som t.ex. urin- och avf\u00f6ringsinkontinens<sup>6-8<\/sup>, men h\u00e4r finns det ocks\u00e5 st\u00f6rre studier som inte finner statistiska samband<sup>5<\/sup>. Urinl\u00e4ckagens storlek, snarare \u00e4n frekvensen, f\u00f6refaller f\u00f6rs\u00e4mra h\u00e4lsorelaterad livskvalitet<sup>6<\/sup>. Avf\u00f6ringsinkontinens \u00e4r korrelerat med att inte ha arbete<sup>9<\/sup>, vilket indirekt kan vara associerat med l\u00e4gre h\u00e4lsorelaterad livskvalitet. Problem med exekutiv funktion, mer \u00e4n intellektuell funktionsniv\u00e5, \u00e4r associerad med reducerad h\u00e4lsorelaterad livskvalitet<sup>10<\/sup> . Sociala aspekter s\u00e5som personlig ekonomi och att inte ha en partner\/sexliv \u00e4r ocks\u00e5 indirekt f\u00f6rknippade med l\u00e4gre (h\u00e4lsorelaterad) livskvalitet<sup>2,3<\/sup>. Ensamhet beskrivs ofta<sup>11,12<\/sup>. H\u00e4lsorelaterad livskvalitet minskar med tiden, m\u00f6jligen sker detta vid yngre \u00e5lder hos vuxna med ryggm\u00e4rgsbr\u00e5ck \u00e4n hos andra<sup>13<\/sup>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"font-size:18px\">\u00d6verlevnaden beror till stor del p\u00e5 kvaliteten p\u00e5 den f\u00f6rebyggande och behandlande v\u00e5rden, s\u00e5 idag g\u00e5r det inte att bed\u00f6ma f\u00f6rv\u00e4ntad livsl\u00e4ngd vid ryggm\u00e4rgsbr\u00e5ck. En stor amerikansk studie har nyligen visat att 34% av alla v\u00e5rdtillf\u00e4llen p\u00e5 sjukhus f\u00f6r vuxna med ryggm\u00e4rgsbr\u00e5ck orsakas av tillst\u00e5nd som kunde varit m\u00f6jliga att f\u00f6rebygga. Av de v\u00e5rdtillf\u00e4llen d\u00e4r patienten avled var det 36% som hade orsakats av tillst\u00e5nd som kunde varit m\u00f6jliga att f\u00f6rebygga <sup>14<\/sup>.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>\u00d6verlevnad<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"font-size:18px\">Mortaliteten vid ryggm\u00e4rgsbr\u00e5ck \u00e4r h\u00f6gst under de f\u00f6rsta levnads\u00e5ren<sup>15<\/sup>. En amerikansk rapport fr\u00e5n b\u00f6rjan av milleniet beskrev att 75% av personer med ryggm\u00e4rgsbr\u00e5ck kunde f\u00f6rv\u00e4ntas uppn\u00e5 vuxen \u00e5lder<sup>16<\/sup>. Nyare l\u00e5ngtidsuppf\u00f6ljningar om \u00f6verlevnad vid \u00f6ppna ryggm\u00e4rgsbr\u00e5ck \u00e4r mindre optimistiska; mortaliteten i en brittisk kohort f\u00f6dda med \u00f6ppet ryggm\u00e4rgsbr\u00e5ck 1963-71 forts\u00e4tter vara h\u00f6gre \u00e4ven i vuxen \u00e5lder (det vill s\u00e4ga att man d\u00f6r tidigare) \u00e4n i \u00f6vriga populationen<sup>17<\/sup>. I en senare l\u00e5ngtidsuppf\u00f6ljning av denna kohort rapporterades tio g\u00e5nger h\u00f6gre risk att d\u00f6 i f\u00f6rtid j\u00e4mf\u00f6rt med kontrollgruppen<sup>19<\/sup>, siffror som \u00e4ven st\u00f6ds av amerikanska data av personer med ryggm\u00e4rgsbr\u00e5ck<sup>15<\/sup>. I den brittiska studien betonades att m\u00e5nga av d\u00f6dsfallen var pl\u00f6tsliga och helt ov\u00e4ntade. Senare kohorter av vuxna med ryggm\u00e4rgsbr\u00e5ck f\u00f6refaller dock ha l\u00e4gre risk att d\u00f6 i f\u00f6rtid \u00e4n \u00e4ldre kohorter18, f\u00f6rmodligen till f\u00f6ljd av b\u00e4ttre behandling.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"font-size:18px\">M\u00e5nga d\u00f6dsfall orsakas av tillst\u00e5nd som kan vara m\u00f6jliga att f\u00f6rebygga. Vad som kan uppfattas som vaga symptom s\u00e5som huvudv\u00e4rk, ont i nacken, snabb synf\u00f6rs\u00e4mring, tryck \u00f6ver br\u00f6st eller mage m\u00e5ste tas p\u00e5 st\u00f6rsta allvar i denna population<sup>17<\/sup>. H\u00f6ga br\u00e5ck \u00e4r associerade med h\u00f6gre d\u00f6dlighet \u00e4n l\u00e5ga br\u00e5ck<sup>20,21<\/sup> och detta samband \u00e4r starkare hos dem som avlidit av d\u00f6dsorsaker relaterade till bl\u00e5sa och njurar<sup>19,21<\/sup>. <br>Hos vuxna med ryggm\u00e4rgsbr\u00e5ck \u00e4r infektioner, hj\u00e4rtsjukdom, njursvikt, traumatiska skador och cancer vanligaste d\u00f6dsorsaker<sup>22<\/sup>. Vid \u201dpl\u00f6tsliga d\u00f6dsfall\u201d i den brittiska kohortstudien beskriven ovan var epilepsi, lungemboli, akut hydrocefalus och uro-sepsis vanligaste d\u00f6dsorsakerna hos vuxna med ryggm\u00e4rgsbr\u00e5ck <sup>17<\/sup>. I den sjukhusbaserade amerikanska studien var den allra vanligaste d\u00f6dsorsaken sepsis, och b\u00e5de stroke och hj\u00e4rtk\u00e4rlsjukdom var vanligare \u00e4n njursvikt<sup>14<\/sup>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"font-size:18px\">F\u00f6rekomst av trycks\u00e5r har associerats med \u00f6kad mortalitet. De som haft trycks\u00e5r i ton\u00e5ren har h\u00f6gre risk att d\u00f6 tidigt \u00e4n de som utvecklat trycks\u00e5r som unga vuxna<sup>23<\/sup>. Amerikanska data p\u00e5 njursvikt i slutstadiet har visat att detta uppst\u00e5r i genomsnitt i yngre \u00e5ldrar hos dem med ryggm\u00e4rgsbr\u00e5ck \u00e4n hos dem utan ryggm\u00e4rgsbr\u00e5ck. Orsaken till njursvikten kunde oftare h\u00e4rledas till tidigare urologiska problem hos dem med ryggm\u00e4rgsbr\u00e5ck \u00e4n hos kontrollgruppen; att ha v\u00e5rdats inom slutenv\u00e5rd p\u00e5 grund av urinv\u00e4gsinfektioner var associerat med h\u00f6gre risk av d\u00f6d vid dialys hos dem med ryggm\u00e4rgsbr\u00e5ck \u00e4n kontrollgruppen med njursvikt i slutstadiet utan ryggm\u00e4rgsbr\u00e5ck<sup>24<\/sup>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"font-size:18px\">I en brittisk studie p\u00e5visades att unga vuxna som f\u00f6ljts kontinuerligt p\u00e5 samma klinik som barn hade l\u00e4gre risk att d\u00f6 i njurkomplikationer \u00e4n tidigare kohorter. Nittiosex procent av dessa patienter f\u00f6dda 2000 eller senare rapporterades ha god njurfunktion (njurskadeklass niv\u00e5 2 eller b\u00e4ttre), vilket kan j\u00e4mf\u00f6ras med 62% hos dem som f\u00f6ddes under 70-talet<sup>25<\/sup>. \u00c4ven svenska data tyder p\u00e5 bra njurfunktion hos yngre kohorter<sup>26<\/sup>.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Rekommendationer<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Prevention<em> <\/em>ska vara ledord f\u00f6r v\u00e5rd och medicinsk uppf\u00f6ljning av vuxna med ryggm\u00e4rgsbr\u00e5ck &#8211; m\u00e5nga sekund\u00e4ra komplikationer och d\u00f6dsorsaker g\u00e5r att f\u00f6rebygga.<\/li>\n\n\n\n<li>Kom ih\u00e5g screening och utredning f\u00f6r \u201dvanliga\u201d tillst\u00e5nd s\u00e5som t.ex. br\u00f6stcancer, h\u00f6gt blodtryck och metabola riskfaktorer.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"font-size:18px\">*Det finns m\u00e5nga definitioner av h\u00e4lsorelaterad livskvalitet och de forskare som refereras h\u00e4r har inte alltid anv\u00e4nt samma definition.
